CRAFFT -- Substance Use Screening for Teens

The CRAFFT consists of six yes/no questions and is meant for adolescents and young adults (roughly ages 12-21). It's recommended by the American Academy of Pediatrics for use during routine checkups. The name is an acronym from the key word in each question: Car, Relax, Alone, Forget, Family/Friends, Trouble.
